Background
Spontaneous intracerebral hemorrhage (sICH) remains a challenging condition in neurosurgery, particularly in patients with antithrombotic medication. These drugs —while vital for preventing thromboembolic events — significantly increase the risk and severity of intracranial haemorrhage. Despite the rising prevalence of antithrombotic drugs, current guidelines for sICH patients focus on non-surgical populations, leaving clinicians with limited evidence to guide urgent reversal strategies and operative planning.
Objectives
The COMAND-ICH (COagulation MAnagement in Neurosurgical Diseases – IntraCerebral Hemorrhage) survey seeks to map global perioperative practices in antithrombotic-medication associated sICH, identify areas of variation, and highlight key uncertainties. Your input will directly inform an international Delphi consensus, ultimately shaping expert-based guidance for individualized coagulation management and supporting future clinical trials.
